2011 Census/Registration and Ministries Survey
| The
Census/Registration and Ministries Survey consists of two parts. The
first part is used for either the census of the NAM parish or for
registration of new parish members. The second part is a survey of
ministry skills within the parish. We request that you fill out both
forms and mail them to the church secretaries.
Part A - Census/Registration Enter your data onto the lines of the form. When you are finished entering the data, print the form from your browser and mail it to the church office at North American Martyrs Church, 2526 Haymaker Road, Monroeville, PA.15146 , or St. Michael Church, 740 Wall Avenue, Pitcairn, PA 15140, or you may drop it in the collection basket on Sunday or bring it to the Rectory/Office. After printing the form, exit from your browser to return to this page for completion of Part B. To enter the Census/Registration form press (click) begin. Part B - Ministries Survey As part of the 2011 NAM census the parish is conducting a survey of skills within the community of members that would assist in the operation of the parish. The survey can be filled out online with your computer's browser. After the completion of the form it can be printed and mailed to the parish secretary at either North American Martyrs Church, 2526 Haymaker Road, Monroeville, PA 15146, or St. Michael Church, 740 Wall Avenue, Pitcairn, PA 15140, or you may drop it in the collection basket on Sunday or bring it to the Rectory/Office. The browsers most commonly used by the NAM Parish members are Internet Explorer (IE) and Firefox (Mozilla). They work a little differently for printing from the browser.Internet Explorer - After the data has been entered in the spaces provided, move the pointer into a blank space. This will highlight "print" in the File dropdown menu. Click on "print". Firefox - After the data has been entered in the spaces provided, move the pointer into a blank space. This will highlight a printer icon at the top of the form. Click on the icon to print the form on your printer. When you are ready, press start survey to bring up the Ministries Survey. After printing, exit the form from your browser's file drop down menu.
